Admission Requirements

Prepare the following documents to ensure a smooth application process. Requirements may vary depending on the program and applicant type.

College Admissions

For Freshmen:

  • Original Form 138 (Report Card)
  • Certificate of Good Moral Character
  • PSA Birth Certificate (Photocopy)
  • 2x2 ID Picture (1 copy, white background)

For Transferees:

  • Honorable Dismissal / Certificate of Transfer Credentials
  • Official Transcript of Records (TOR) for evaluation
  • Certificate of Good Moral Character
  • PSA Birth Certificate (Photocopy)
  • 2x2 ID Picture (1 copy, white background)

Senior High School Admissions

For Incoming Grade 11:

  • Original Form 138 (Grade 10 Report Card / SF 9)
  • Certificate of Good Moral Character
  • PSA Birth Certificate (Photocopy)
  • 2x2 ID Pictures (2 copies, white background)
  • ESC Certificate
  • QVR Certificate

For Transferees (Grade 11 or 12):

  • Transfer Credential
  • Original Form 138(SF 9) from previous SHS
  • Certificate of Good Moral Character
  • PSA Birth Certificate (Photocopy)
  • 2x2 ID Picture (1 copy, white background)

Short Term / TESDA Courses

  • PSA Birth Certificate (Photocopy)
  • 2x2 ID Pictures (4 copies, white background)
  • 1x1 ID Pictures (4 copies, white background)
  • Transcript of Records or Form 137/138 (if applicable)
  • Marriage Certificate (if married, for female applicants)

Enrollment & ID Processing

Present proof of payment to the Registrar's Office for official enrollment and processing of your school ID.
